Appointments:

To schedule an appointment with Dr. Kortz please visit the contact our office page. When calling to make a new patient appointment please have the following information available:   insurance information, name of referring physician and primary care doctor (along with their phone numbers), information on which tests you’ve recently had done and where. In order to provide the best care possible we must know at which facility any imaging has been done at.

We ask that patients show up 10 minutes before their scheduled appointments. For your convenience we have made available the necessary forms needed for your first appointment (click here)

What to Bring

  • We ask that you assist us in serving you by bringing the following items with you to your appointment:
  • Your driver’s license
  • Your medical insurance cards
  • A list of current medications, including dosage information (frequency, strength)
  • Medical records if requested by our office
  • Payment (for your convenience we accept cash, check, Visa, MasterCard, and Discover)

Insurance and Disability Forms

We participate with most major insurance carriers. Please see our list of insurance companies we accept for more information. If you are a self-pay patient, please contact our office for costs. All payments are expected at the time of service.

Disability forms such as FMLA, will incur a charge of $35.00 per form. Payment must be made prior to completion. Please allow up to 2 weeks to have forms completed and signed. Also, please complete any information that is required by you and make sure that your name is on the form.

Questions for Your Surgeon:
